Fill in this form and print it out to send your media to Video Kitchen. The top part helps you determine what we will charge for all the work you select to be done.

Send no money or credit card information with your order. We will contact you by telephone with our final charges and get your payment instructions at that time.

After processing, your original master and the duplicates will be available for pickup unless you advise us differently.

If we determine that the total charges are going to exceed $
we will not run the job without first obtaining your permission.

Normal turnaround is 24 hours for any order under 250 copies.
