Fill in this form and print it out to send your media to Video Kitchen. The top part helps you determine what we will charge for all the work you select to be done.

Send no money or credit card information with your order. We will contact you by telephone with our final charges and get your payment instructions at that time.

After processing, your original photographs plus any DVD's and tapes will be returned by UPS with $100 insurance to the address below unless you advise us differently.

If we determine that the total charges are going to exceed $
we will not run the job without first obtaining your permission.

We do not print digital photos. We can however, save your photos to a disk that you can have printed at a photo kiosk.
